ATM Fee Refund Request

As a courtesy to our members, East Wisconsin Savings Bank offers refunds on ATM transaction fees, up to $20.00 per calendar month! Simply complete and submit our ATM Fee Refund form within 60 days of incurring an ATM fee, and your refund will be on its way! *

Please provide your first and last name, and the last 4 digits of your debit/ATM card number. You may request reimbursement for up to 5 transactions on each form submission. Fees will be refunded if your request is made within 60 days of each transaction, up to a maximum of $20.00 per calendar month.
